Hospice services are designed to provide support to patients, their families and loved ones wherever they call home. This can be a Long Term Care Facility, Assisted Living, Retirement Community or at the patients own home. The goal is to maximize the quality of the patients life.
Our services include Medical Care, skilled nursing care, certified home health aides, social workers assigned to the patient, spiritual counseling, assistance with personal care, pharmaceutical services and more.
The hospice plan of care is individualized to meet each patient’s needs. If special equipment (e.g., wheelchair, lift, hospital bed, etc.) is needed, the hospice provider offers these as part of its services.

Blue Cross Hospice physicians and team members work with your doctor to ensure your clinical and emotional needs are being met and that your care is being carried out appropriately. Your doctor chooses his or her level of participation in your care. Admissions to Blue Cross Hospice is based on the patient's needs rather than the ability to pay. Hospice services are covered by :
